How to fill in a blank invoice
Your business name and contact
The name you're paid under, your address, email and phone. Add a tax ID if your clients ask for one.
Invoice number
A unique number, one higher than your last. Never reuse a number, even for a small job.
Issue date and due date
Write both as dates. Net 30 is clear to you, but a date on the page is clearer to your client.
Bill to
The client's name and billing address, or the person who approves payment at a company.
Description, quantity and rate
One line per product or service, specific enough that the client recognizes the work.
Amount and subtotal
Multiply quantity by rate for each line, then add the lines together.
Tax and total due
Add tax only if you charge it, then write the total due as one clear figure.
Notes and payment terms
A short thank-you, when payment is due and how to pay you.

Tips for people starting from a blank invoice.
Keep one copy blank
Save the blank invoice template as your master and work on copies. Whether you type or print, there's always a clean page to start from.
Number it before anything else
Write the invoice number first and note it in your records. Gaps and duplicates are easier to prevent than to explain.
Pick the version that does the work
